Your "respondent ID" is a combination of your last name and the last 4 digits of your student ID, *without* any spaces in between. 
For example, it might be "smith1234" or
"jones6789".

The login requirement ensures that only USCL students respond and do it only once.  The surveys will be treated as anonymous.
