Your "respondent ID" is a combination of your last name and the last 4 digits of your SSN, *without* any spaces in between. 
For example, it might be "smith1234" or
"jones6789".

The login requirement ensures that only USCL faculty respond and do it only once each. 

The surveys will be treated as anonymous.
