Financial Aid Forms

Below is a directory of forms that you may be required to complete as part of the Financial Aid process. These PDF forms can be completed online and then printed. The information you add will not save when you close the form, so be sure to review your printed form before closing the document.  You can also print a blank document and complete it by hand.

When you complete the form, print and sign it before submitting it to the campus Financial Aid Office.  If you have questions please contact the Financial Aid Office by phone at 803-313-7068 or by e-mail at .

Application for Federal Student Aid (FAFSA)

You must file the Free Application for Federal Student Aid (FAFSA) in order to be considered for most types of financial aid.

Asset Information Form

If you reported Assets on either your FAFSA or other documentation that did not match information already on file, or if we have information that suggests you have other assets you have not reported, you may be asked to complete this worksheet.  If so, complete and return this form to our office.

Authorization to Apply Current FA to Prior Charges

If you have a bill from a prior academic year, you can not normally use your current year financial aid to pay that bill.  You may complete this form if you want to authorize the Business Office to apply your current Financial Aid to pay that prior bill (all current year charges must be paid first).

Authorization to Award Federal Student Aid

If you have been awarded Financial Aid at more than one institution of Higher Education during the current academic term, you may need to complete this form to authorize the USC Lancaster Financial Aid Office to award you your Federal Student Aid.

Authorization to Release Financial Aid Information

If you want the USCL Financial Aid Office to release information about your Financial Aid to another agency, you must complete this form.

Change of Campus for Financial Aid

If you are attending another USC campus, but you initially authorized USCL to award your Financial Aid, you must complete this form to change that authorization.

Change of Dependency Form

If you do not meet the requirements to be considered an Independent student, but you are unable to attain your parents’ information due to unusual circumstances you may appeal to have your dependency status re-evaluated.   

Decline Financial Aid Award

If you wish to decline one or all of your Financial Aid Awards, complete this form and return it to our office.

Degree Status Form

If there is question as to the status of your college degree, you can complete a Degree Status Form to clarify or correct this information.

Direct Deposit Form

12-13

Have your financial aid overage checks directly deposited into your bank account.  Do this at least two weeks before your overage checks are available

Direct Parent PLUS Loan Borrower Information

Direct Stafford Loan Entrance Counseling

Direct Stafford Loan Exit Counseling

Direct Stafford Loan Master Promissory Note

Household Size Update Form, Dependent

If the number of people in your parents’ household has changed since the time that you filed your FAFSA, you must notify the Financial Aid Office.  You can do so using this form.

If the number of people in your household has changed since the time that you filed your FAFSA, you must notify the Financial Aid Office.  You can do so using this form.

Housing Status Form

If your housing status (where you will live while attending college) is in question, you may need to complete a Housing Status Form to update or correct it.

Income Exclusions Form

If you indicated on your FAFSA that one or more of the income exclusions applies to you and this information is in question, you may need to complete the Income Exclusions Form.

Legal Dependent Support Form

12-13

If you indicated that you have a child or other dependent whom you provide at least half of the support for, but you are under the age of 24, you may be asked to complete this form.  If so, complete and return this form to our office with all supporting documentation.

LIFE Scholarship Affidavit

If you are a LIFE Scholarship recipient, you must read and complete this affidavit to receive your scholarship.  You must also read the Terms and Conditions on VIP.

Lottery Tuition Assistance Certification

12-13

If you are a Lottery Tuition Assistance recipient, you must read and complete this affidavit to receive your grant.  You must also read the Terms and Conditions on VIP.

 Lottery Tuition Assistance Application for Dually Enrolled High School Students

If you are a high school student taking at least six hours of college courses at USCL, and are a south Carolina resident, you may qualify for Lottery Tuition Assistance.  Complete and return this form to our office.

Marital Status Form, Dependent

If your parents’ marital status is in question, you may need to complete a Marital Status Form to update or correct it.

Minimum Hours Adjustment Request

If you are taking less than full time (<12 hours), or if you are changing the number of hours you are enrolled for, you must inform the Financial Aid Office so your awards can be adjusted.  Complete and return this form.

Non-parental Information Form

If you filed your FAFSA using information that is not that of your parents’ you may be asked to complete a Non-Parent Information Form.

Perkins Loan Indebtedness Statement

If you have Perkins loans you must complete a Loan Indebtedness Statement.

If you have been awarded a Federal Perkins Loan these are the instructions on what you need to do to receive the funds.

Private Loan Certification Form

If you are borrowing a loan from a private lender (not a Direct Stafford Loan), then you must complete this form.

Satisfactory Academic Progress (SAP) Appeal

Transfer Form

If you are a South Carolina Need Based Grant recipient and have attended classes at another public college in South Carolina, you must complete this form and provide it to the financial aid office at your previous college.  They must return it to us signed, even if you did not receive the grant at their college.

If you have already applied for financial aid, but have had a significant change in your family income since the prior tax year, you may complete this form to request an adjustment to your application information.

State Military Dependents Form

Non-Resident students who qualify to pay in-state tuition due to being a dependent of an active duty military service member must complete this form and return it to the Financial Aid Office in order to be eligible for State Scholarship Funds.

Summer: Financial Aid Application Update

If you intend on taking Summer Classes between May and July of 2013, and wish to be awarded financial aid, you must complete this application update to tell us how many hours you are taking and if you wish to use the remainder of your loan eligibility (if any).

Students receiving Veterans GI Bill benefits must request certification of their enrollment.  The Financial Aid Office will need a copy of one of the following: DD-214, NOBE, or Certificate of Eligibility.  Certification Requests should be submitted at least 30 days prior to the beginning of classes. Summer Financial Aid Application

If you intend to take courses during the Summer semester, you must notify the financial aid office by completing this form.  The form becomes available around the end of February.

TAA Budget and Book Worksheets

If you are a Trade Investment Act recipient you must complete this two page form and return it to this office every semester to receive your funding.

Transfer Student Form

If you need to report colleges you may have attended in the past, you can complete the Transfer Student Form.

Verification, IRS Data Retrieval Instructions

If you were selected for verification, you may provide your IRS Tax Information through the IRS Datamatch process.  Follow these instructions. 
Don’t forget to also complete the proper Verification Worksheet.
