Outreach

The Diabetes Education Clinic hosts three support groups each month.

Evening Diabetes Support Group: 1st Thursday of every month at 6:00 pm
Morning Diabetes Support Group:   2nd Tuesday of every month at 10:00 am
Weight Loss Support Group:  3rd Tuesday of every month at 10:00 am

What We Do

The Diabetes Education Program of USC Lancaster Health Services provides education for persons with diabetes and their families which enables them to:

  • Take an active role in self-management of diabetes.
  • Implement preventive measures to decrease complications associated with diabetes.
Education

Education is provided through individual and small group sessions in a relaxed setting with a focus onProject LAUGH 2006

  • Physiology of diabetes
  • Signs and symptoms of diabetes
  • Current treatments for diabetes
  • Caring for yourself and others
  • Healthy living for diabetes
  • Nutritional approach to managing diabetes
  • Food selection, meal-planning, & preparation
  • Role of family in diabetes care
  • Prevention of complications
  • Stress management and effective coping with chronic illness
  • Insulin pump training
  • What is the difference in high blood sugar and low blood sugar and how to avoid them
Providing Services For
  • Adult: Type 1 and Type 2 Diabetes
  • Pediatric: Type 1 and Type 2 Diabetes
  • Gestational Diabetes
  • Diabetes in Pregnancy
  • Insulin Pump Therapy
  • Nutritional Counseling
  • Continuous Glucose Monitoring System

  • Improved self-management skills of patient and family members

  • Stable blood glucose levels
  • Decreased complications from diabetes
A Team Approach

Utilizing a team approach to diabetes education, services are provided by the following persons:

  • Clinical Dietitian
  • Exercise Physiologist
  • Nurse Educator
  • Nurse Practitioner
  • Certified Diabetes Educator
  • Medical Director
  • Program Director
Information

Appointments can be made through a referral from your Primary Care Provider.

We accept private insurance, Medicare, & Medicaid. For certain private insurance plans, pre-certification may be required. Sliding scales fees are available to those who qualify.

Persons with diabetes are seen in individual and/or group sessions, including family members as appropriate. National standards of the American Diabetes Association are the cornerstone of our educational sessions. The Diabetes Education Clinic is certified through the American Diabetes Association.

Monthly Diabetes support groups are available in the morning and evening.
